Me3 – Paragraph 4 or 5

Me3 is usually the 4th or 5th Paragraph. 

  • Me3 is not mandatory.
  • Tense: Past tense with “had.”
  • Usually includes past medical history.

Now, look at a short case notes:

Scenario: Emily Johnson was admitted on 01/02/2025 with severe abdominal pain and nausea. She was diagnosed with acute appendicitis and underwent a laparoscopic appendectomy. Her recovery has been smooth, with stable vitals and no post-surgical complications. She was responding well to pain management till yesterday, but suddenly she has pain in the surgical site. You are transferring the patient to a gastroenterologist.

Notes:

  • 2018 – Tonsillectomy for chronic tonsillitis.
  • 2019 – Diagnosed with hypertension; controlled with medication.
  • 2020 – Treated for urinary tract infection with antibiotics.
  • 2022 – Knee arthroscopy for meniscus tear.
  • 2023 – Episode of acute bronchitis; resolved with bronchodilators.

Me3 Sample Paragraph

Please note, Ms Johnson underwent tonsillectomy for chronic tonsillitis in 2018. In 2019, she was diagnosed with hypertension which is being controlled with medication. After a year, she was treated for urinary tract infection with antibiotics.
