Sample Case Notes
Emily Johnson was admitted on 01/02/2025 with severe abdominal pain and nausea. She was diagnosed with acute appendicitis and underwent a laparoscopic appendectomy. Her recovery has been smooth, with stable vitals and no post-surgical complications. She is responding well to pain management and is expected to be discharged soon.
Past Medical History
- 2018 – Tonsillectomy for chronic tonsillitis.
- 2019 – Diagnosed with hypertension; controlled with medication.
- 2020 – Treated for urinary tract infection with antibiotics.
- 2022 – Knee arthroscopy for meniscus tear.
- 2023 – Episode of acute bronchitis; resolved with bronchodilators.

Ms Johnson underwent tonsillectomy in 2018. She was diagnosed with hypertension a year later which is being controlled with medication. In 2020, she was treated for urinary tract infection and after two years, she had a knee arthroscopy for meniscus tear. Most recently, she had an episode of acute bronchitis in 2023 and was resolved with bronchodilators.

John Smith was admitted on 03/15/2025 with chest pain and shortness of breath. He was diagnosed with a myocardial infarction and underwent percutaneous coronary intervention (PCI). His condition has stabilized, and he is currently undergoing cardiac rehabilitation to improve his cardiovascular health. His exercise tolerance has improved, and his risk factors are being managed.
Past Medical History:
- 2017 – Coronary artery bypass graft (CABG)
- 2018 – Type 2 diabetes mellitus; managed with metformin
- 2019 – Hyperlipidemia; on statins

Michael Torres was admitted on 04/05/2025 with complaints of joint pain and swelling. He was diagnosed with rheumatoid arthritis and started on methotrexate and physiotherapy. His symptoms have moderately improved, but long-term management is required.

- 2019 – Diagnosis of hypothyroidism; on levothyroxine
- 2020 – Left shoulder rotator cuff repair
