OET Writing Me3 Tasks

Me3, or Paragraph 4 is optional. It is required when:

• the patient is not diagnosed, 
• when the patient is unknown to the recipient, and, 
• when the PMH is provided.

NB: Include only details relevant for the recipient. 

• Me3 is Past Medical. Use only past tense. 

Sample Case Notes

Emily Johnson was admitted on 01/02/2025 with severe abdominal pain and nausea. She was diagnosed with acute appendicitis and underwent a laparoscopic appendectomy. Her recovery has been smooth, with stable vitals and no post-surgical complications. She is responding well to pain management and is expected to be discharged soon.

Past Medical History

• 2018 – Tonsillectomy for chronic tonsillitis.
• 2019 – Diagnosed with hypertension; controlled with medication.
• 2020 – Treated for urinary tract infection with antibiotics.
• 2022 – Knee arthroscopy for meniscus tear.
• 2023 – Episode of acute bronchitis; resolved with bronchodilators.

Answer

Ms Johnson underwent tonsillectomy in 2018. She was diagnosed with hypertension a year later which is being controlled with medication. In 2020, she was treated for urinary tract infection and after two years, she had a knee arthroscopy for meniscus tear. Most recently, she had an episode of acute bronchitis in 2023 and was resolved with bronchodilators.

Me3 Sample 2

John Smith was admitted on 03/15/2025 with chest pain and shortness of breath. He was diagnosed with a myocardial infarction and underwent percutaneous coronary intervention (PCI). His condition has stabilized, and he is currently undergoing cardiac rehabilitation to improve his cardiovascular health. His exercise tolerance has improved, and his risk factors are being managed.

Past Medical History:

• 2017 – Coronary artery bypass graft (CABG)
• 2018 – Type 2 diabetes mellitus; managed with metformin
• 2019 – Hyperlipidemia; on statins
• 2021 – Acute pancreatitis; resolved with conservative management
• 2022 – Cataract surgery, left eye.

Answer

Mr Smith underwent coronary artery bypass graft in 2017 and cataract surgery for his left eye in 2022. He was diagnosed with diabetes melitus, hyperlipidemia and acute pancreatitis in 2018, 2019 and 2021, respectively. The latter was resolved with conservative management.

Michael Torres was admitted on 04/05/2025 with complaints of joint pain and swelling. He was diagnosed with rheumatoid arthritis and started on methotrexate and physiotherapy. His symptoms have moderately improved, but long-term management is required.

Past Medical History:

• 2017 – Appendectomy
• 2019 – Diagnosis of hypothyroidism; on levothyroxine
• 2020 – Left shoulder rotator cuff repair
• 2021 – Anemia; treated with iron supplements
• 2023 – Gout flare; managed with NSAIDs

Answer

Mr Torres underwent an appendectomy in 2017. He was diagnosed with hypothyroidism after two years and commenced management on levothyroxine. He underwent a left shoulder rotator cuff repair in the following year and was diagnosed with anemia for which he started taking iron supplements. Lastly, in 2023 he reported a gout flare up which was managed with NSAID.

Me3 Sample 1

Sarah Lee was admitted on 02/10/2025 with complaints of severe headaches and dizziness. She was diagnosed with migraine with aura and was started on sumatriptan and lifestyle modifications. She has shown improvement with fewer migraine episodes and better symptom control.

• 2016 – Diagnosed with asthma; on inhalers
• 2018 – C-section delivery
• 2020 – Gallbladder removal (cholecystectomy)
• 2022 – Allergic rhinitis; on antihistamines
• 2024 – Vitamin D deficiency; corrected with supplements.

Me3 Sample 2

Laura Kim was admitted on 05/12/2025 with persistent cough and fever. She was diagnosed with community-acquired pneumonia and treated with IV antibiotics. She is now afebrile and showing signs of recovery, with discharge planned in two days.

• 2015 – Hysterectomy for fibroids
• 2017 – Breast biopsy; benign findings
• 2018 – Hyperthyroidism; treated with radioactive iodine
• 2020 – Lumbar disc herniation; resolved with physiotherapy

David Brown was admitted on 06/22/2025 with sudden vision loss in the right eye. He was diagnosed with retinal detachment and underwent vitrectomy and laser therapy. His vision has partially improved, and he is scheduled for follow-up with ophthalmology.

• 2018 – Chronic kidney disease stage 2
• 2019 – Skin melanoma excision
• 2021 – Osteoarthritis in knees; on NSAIDs

Me3 Sample 4

Olivia Patel was admitted on 07/10/2025 with severe lower back pain. She was diagnosed with lumbar spinal stenosis and received an epidural steroid injection. Her pain has reduced, and she is ambulating with support. Physical therapy has been initiated for long-term pain management.

Past Medical History:

• 2016 – Preeclampsia during pregnancy
• 2017 – Migraine; managed with propranolol
• 2019 – Vitiligo; under dermatology care
• 2020 – COVID-19 infection; mild symptoms
• 2023 – Uterine polyp removal

Daniel Wang was admitted on 08/18/2025 with difficulty breathing and wheezing. He was diagnosed with a COPD exacerbation and treated with bronchodilators and corticosteroids. His respiratory status has improved, and he has been advised on smoking cessation and pulmonary rehabilitation.

• 2017 – Diagnosed with GERD; on proton pump inhibitors
• 2018 – Sleep apnea; using CPAP machine

Sophia Martinez was admitted on 09/01/2025 with persistent fatigue and pallor. She was diagnosed with iron deficiency anemia and received IV iron therapy. Her hemoglobin levels are improving, and she has been provided dietary counseling to prevent recurrence.

Me3 Sample 7

James Green was admitted on 10/05/2025 with severe leg swelling. He was diagnosed with deep vein thrombosis (DVT) and started on anticoagulation with warfarin. His swelling has reduced, INR is being monitored, and compression stockings have been prescribed for ongoing care.
