Notes:

Today’s Date: 21 July, 2024.

You are a school nurse at the boarding school where Marie Winfield is a 3rd Grade Student. She fell down in the school and is being monitored.

  • Patient: Marie Winfield.
  • Age: 8 years.
  • Address: Resident scholar at Almond County School, Ottawa North. She has special permission to go home on all Fridays (her home is only 38 miles from school).

Incident Details:

  • Date of Incident: 18 July, 2024.
  • Location: Basketball court at Almond County School.
  • Possible cause: Overstraining and/or exposure to high temperature (37°C)

Symptoms/Condition Post-Fall:

  • Dizziness and weakness immediately following the fall.
  • Mild headache.
  • Fatigue over the past three days.
  • Complaints of persistent urination and bloating.

Immediate Care Provided:

  • Hydration with oral rehydration solution (ORS).
  • Application of ice pack to minor bruises.
  • Discussed the incidents with her family physician Dr John Mortimer.

Medications Administered:

  • Paracetamol (Tylenol) 250mg: For pain relief and fever management.
  • Ibuprofen (Advil) 200mg: For reducing inflammation and managing pain

Social Details:

  • Resident scholar, actively participates in school sports and extracurricular activities.
  • Known to be cheerful and sociable among peers.
  • No stress or anxiety reported (however, close friends reported a sudden change in her behavior).
  • Parents (both) running business.
  • Residential Address: 3456 Carl Island, Ottawa East – United States.

Academic Performance:

  • Marked drop in class test performance over the past three weeks.

Having symptoms suggestive of:

  • Heat exhaustion.
  • Dehydration.
  • Possible urinary tract infection (UTI).
  • Potential underlying anemia or other systemic conditions.

Information collected from parents:

  • No issues at home. Financially doing better.
  • Marie had attended the funeral of her grandmother a month ago. “Was attached to Grannie.”

We suspect that her current condition has been partially or completely caused by her grandmother’s demise since she was very attached to her.

Follow-Up Actions:

  • Monitor hydration levels and encourage regular fluid intake.
  • Avoid strenuous activities and direct sunlight exposure until fully recovered.
  • Schedule a comprehensive medical evaluation to rule out other underlying conditions, including a urine test to check for UTI and blood tests for anemia

Plan:

  • Allow Marie to spend time with family for the next 2 weeks.
  • Rest and avoid physical exertion for the next 48 hours.
  • Maintain a high fluid intake to ensure proper hydration.
  • Administer Paracetamol and Ibuprofen as needed for pain management.
  • Follow-up appointment with the family physician for further evaluation, including urine and blood tests (appointment scheduled for 24 July, Monday).

Writing Task

Using the information in the case notes, write a letter to Marie’s parents highlighting the recent developments. Include all relevant information.

In your answer:

  •       Expand the relevant notes into complete sentences.
  •       Do not use note form.
  •       Use letter format.

The body of the letter should be approximately 180–200 words. However, you needn’t include any irrelevant information to reach the word-count; nor do you have to omit any information to avoid exceedance.
