Re: Mr Tom Clarke; DOB: 21 September, 1954

Mr Tom Clarke requires home visits for rehabilitation program. He has stiffness and limited range of motion in the left elbow following an injury.

Mr Clarke was presented to the emergency department following an accident on 15th February, 2023 and was diagnosed with non-displaced fracture of the coronoid process of the ulna, marrow oedema on the head and neck of the radius involving the articular surface along with moderate joint effusion. The condition was managed with medication and the elbow immobilized with a sling for 6 weeks.

Mr Clarke re-visited on 11 July for x follow up and his X-ray revealed a well healing injury. The sling was taken off and an exercise program was initiated at his home.

Since Mr Clarke has the same complaints during today’s visit, it would be appreciated if you could visit him at his home to provide rehabilitation program. Mr Clarke lives with wife at 92 Lygon Street, Carlton and his contact number is 0422-894-896. Of note, his next appointment will be on 20 September, 2022.
