Dear Dr Robinson,
Ref: Mr Edwin Bruker, 56 years
Mr Edwin Bruker _____ (require / requires) further assessment and management since he _____ (has been / is) is suspected to _____ (have / be) post stroke depression.
Initially, Mr Bruker was admitted on 14 February, 2021 with symptoms suggestive of stroke and was diagnosed with acute ischemic stroke for which thrombolysis was performed and aspirin was commenced. Also, he was referred to a speech therapist. Although he was anxious about recovery, Mr Bruker was discharged with an improving condition.
Mr Bruker was transferred to my care with ongoing physiotherapy. In addition to speech therapy and supportive care, reference to a dietitian was made since (because) he has moderate dysphagia. This was followed by a period of progress.
On 1 May, 2021 Mr Bruker was noticed with an onset of depression, poor appetite along with weight loss and poor grooming. Currently, he has noncompliance to medication coupled with lack of interest in physiotherapy. Please note, most of the time he stays inside the room and has no interaction with others. Therefore, cognitive behavioural therapy and daily counselling sessions have been initiated.
In the light of what has been mentioned above, it would be appreciated if you could provide continued care to/for Mr Bruker. Kindly note, he has worsening symptoms such as paranoid behaviour, delusion ____ and insomnia. May I remind you that he once attempted suicide and his family was already informed regarding his condition.
