Dear Dr Mona,
I hereby refer Ms Rita for further assessment and management owning to her semiconciousness probably because of medication overdose.
Today,during morning assessment Ms Rita was found semi conscious and her breathing was shallow .In addition,2 ampoules were found next to her bed which were labelled in spanish.She seems to have self harm as she was concerned about euthanasia. The doctor on duty has been informed about the same and as instructed photographs of the ampoules has been taken.
In view of the above,it would be appreciated if could evaluate her condition and provide treatment as appropriate

I am writing to refer Ms Rita who is under our care is being found semiconscious at 8 .In addition to this , two empty ampoules of some medicine was found near her , which was labelled in Spanish language .Subsequently, her GP has been informed and he has suggested for an urgent evaluation of her present condition at your facility .
